Bontoc, Mountain Province – The Provincial Local Government Unit of Mountain Province in partnership with the Department of Foreign Affairs (DFA) will be conducting a mobile passport processing on April 5, 2019 from 8:00 a.m. to 4:00 p.m. as part of the week-long celebration of the 15th Lang-ay Festival and 52nd Founding Anniversary of Mountain Province come April 1-7, 2019.
The services to be offered are renewal of expired passports and processing of new passports. This is in line with the provincial government’s program for employment/ job-bridging program through supporting its constituents’ wish or desire to go abroad.
According to Governor Bonifacio C. Lacwasan, Jr., this is the third time under his administration that the DFA was invited to provide faster processing services within the province to minimize the expenses, time and efforts of those who want to apply for or renew their passports.
“We are yearly requesting the DFA- Office of Consular Affairs for slots, updates and guidelines to facilitate the passport services activity in the province because we believe that this is relevant and has a big impact on employment opportunities,” he said.
Meanwhile, Executive Assistant and Focal Person for Labor and Industry Adelina Jocelyn F. Cullaling said that the DFA has allotted about 500 slots for Mountain Province while the LGU is also extending its services to those from the nearby provinces who wish to avail of the mobile passport processing.
She said that the filling up of the basic application form is ongoing at the Provincial Governor’s Office until the end of March. The duly-signed forms shall be forwarded to the DFA for verification.
Here are the requirements for the regular passport application in time for April 5.
1. Adult new applicant: personal appearance, accomplished application form, Original copy of Philippine Statistics Authority (PSA) Birth Certificate on security paper; married females must also submit original copy of NSO/PSA Marriage Contract on Security Paper or NSO/PSA Report of Marriage and original and photocopy of valid IDs.
2. Adult renewal applicant: A. Renewal of Machine Readable Passports (Maroon), Green Passports and any older passports will be treated as a new application. Requirements – personal appearance, accomplished renewal application form, original copy of PSA Birth Certificate on Security Paper, Original Copy of PSA Marriage Contract (for Married Females) and original and photocopy of valid IDs.
B. Renewal of ePassports – Accomplished renewal application form, personal appearance, current ePassport with photocopy of data page and original PSA documents that will support the change of name (if applicable): Marriage Contract, Annotated Birth Certificate, Annotated Marriage Contract and Death Certificate of Spouse.
3. Minor new applicant: accomplished application form, personal appearance of minor applicant and either parent or authorized adult companion with compete requirements (SPA and Affidavit of Support and Consent), PSA Birth Certificate, Passport or Valid ID of either parent and school ID (8 years old and above).
4. Minor renewal applicant: accomplished application form, personal appearance of minor applicant and either parent or authorized adult companion with complete requirements (SPA and Affidavit of Support and Consent), PSA Birth Certificate, Passport or Valid ID of either parent, current passport of the applicant and school ID (8 years old above)
Moreover, the list of acceptable IDs are SSS/GSIS Unified Multi-Purpose Identification (UMID) Card, LTO Driver’s License, PRC ID, OWWA/iDOLE card, Comelec voter’s ID/ Voter’s Registration Record from Comelec Head or Regional Office, PNP Firearms License, Senior Citizen ID, School ID (for students) and Current Valid ePassport (For Renewal of ePassport). **Erwin S.
